If you wish to submit a reimbursement request to the PTSA please follow the steps below:

  • Take photos of your receipts.
  • Download and complete the following reimbursement request form.
  • Email both the completed form and the photos of the receipts to treasurer@hooverptsa.org.
  • We will reply to your email acknowledging receipt of your request.
  • The request will be reviewed by the PTSA President and Treasurer.
  • We will send email confirmation to you when the request has been approved and the reimbursement check has been issued.
